Your appointment is very important to us at Reborn Medical Aesthetics. It is reserved specifically for you.
Please understand that when you forget or cancel your appointment without giving enough notice, we miss the opportunity to fill that appointment time, and clients on our waitlist miss the opportunity to receive services.
We understand that sometimes schedule adjustments are necessary; therefore, we ask that if you must cancel or reschedule your appointment, please give us 48 hours’ notice or more. If you do not provide us with the allotted time or no-show, you will be charged a $50 non-refundable fee.
To start booking, click on book now button and it will direct you to our booking site. Select the treatment options from the menu on the booking site. A credit card is required to hold all appointments.
Once you have completed the booking, please fill out the medical assessment form that will be emailed to you later.
Please indicate on your intake form and inform your practitioner of all medical conditions. All clients must undergo a medical consultation with our nurse practitioner prior to receiving prescription medication.
If you are a first-time client; a consultation is free and complimentary, can be booked in advance if you have questions or require more information on any procedures. OR this will be done on the same day during your treatment appointment. Consultations must be renewed annually or if you are doing a new treatment area
Your guests are more than welcome to come with you for the duration of your appointment
Prices and services are subject to change at any time.
Service packages are final sale (non-refundable) and non-transferable.
Clients are not required to wear a mask to their appointments. Your injector will continue wearing a mask during your treatment.
